Care is individualized to meet the needs of each patient and family unit, focusing on comfort and stressing human values that go beyond physical needs when a traditional approach to disease is no longer desired or appropriate. It is our belief that dying can be accepted as a unique part of life. Each hospice patient receives care by a team of trained hospice professionals and dedicated hospice volunteers. The hospice team works closely with the patient’s physician with attention paid to each individual patient’s needs. Emphasis is given to controlling distressing physical symptoms, as well as other concerns the patient or family may have. Hospice care may be delivered wherever the patient calls home, including private homes, assisted living facilities, nursing homes, board and lodging facilities, foster care or the Seasons Hospice House. Seasons Hospice affirms and cherishes life, helping each person to live each day to the fullest extent possible, adding life to days when days can no longer be added to life. |
