Mission and Vision
The mission of Seasons Hospice is to provide individualized, comprehensive and compassionate care, education and support for the dying, their loved ones and others who have experienced loss.
Adding quality of life to each day is the vision of Seasons Hospice and this vision is upheld with every interaction between our staff and patients, their loved ones and our bereavement clients.
The Seasons Hospice Story
Since the early 1990’s, Seasons Hospice has been providing hospice services in the Rochester, Minnesota area to persons who are living with terminal illness, who no longer seek curative treatment. Originally part of Shamrock In-Home Nursing, Seasons became an independent non-profit agency, certified both by Medicare and Medical Assistance, in the Spring of 1996.
In November 1997, the Seasons Hospice House was opened in southwest Rochester, representing the first Medicare certified hospice facility in the state of Minnesota.
Our Service Area
Seasons Hospice provides hospice and bereavement care to individuals and families within Rochester and a 30 mile radius around the city encompassing seven counties. These include: Dodge, Fillmore, Goodhue, Mower, Olmsted, Wabasha and Winona counties.
Care Team
Each patient and family on our program work together with a personalized care team to create their individualized plan of care which embraces a holistic approach. This team includes:
- Skilled registered nurses and patient care coordinators, also registered nurses, provide expertise in managing patients’ physical symptoms and support patients and families whenever needed.
- Licensed practical nurses provide routine personal care and assistance in a variety of ways.
- Licensed social workers give social and emotional support, counseling and referral services for community programs.
- Licensed and board certified music therapist uses music within a therapeutic relationship to achieve non-music goals.
- Certified massage therapist is available for patients residing in their own homes or in facilities as well as at the Hospice House.
- Chaplain provides non-denominational spiritual support to patients and families whenever the need for spiritual support or counseling arises.
- Grief counselors offer bereavement support to patients’ loved ones during the time patients are on our hospice program and up to 13 months following the loss of a loved one. Bereavement services are offered to members of the general community as well.
- Medical doctors are available 24 hours a day, seven days a week to consult with nursing staff and other physicians.
The Seasons Hospice staff works closely with patients’ physicians and other medical professionals to ensure continuity of care. Highly trained volunteers also are an integral part of the Seasons Hospice care team, offering assistance to patients and their families in a variety of ways.
Leadership Team
Seasons Hospice is governed by a board of directors who meet bi-monthly and advise the staff on policy issues and provide fiduciary oversight. They represent both consumers who have used our services, and members of the medical and business communities with expertise in hospice care, finance, law, marketing, and fund development. The executive committee works closely with the executive director on administrative matters.
Referrals
Referrals to Seasons Hospice can be made by physicians or other health care professionals, family members, friends, clergy or the person seeking care. The referral process begins with a phone call to Seasons Hospice recommending an individual for hospice care. Our administrative office telephone number is 507-285-1930, between the hours of 8:30 a.m. to 5:00 p.m., Monday through Friday. After receiving a referral, Seasons Hospice contacts the prospective patient, representative or health care agent to discuss their interest in hospice. Our goal is to make this contact within 24 hours of receiving the referral. Individuals are at no time obligated to sign onto the Seasons Hospice program, and as always, Seasons upholds strict confidentiality measures during the inquiry process.
If the prospective patient wishes to consider hospice care with Seasons Hospice, we then contact his or her physician and assess eligibility. The final step in the referral process is arranging for a meeting with the prospective patient and family to discuss care needs and how Seasons Hospice can help.
