Home Hospice Care

What services are provided by hospice?
The vast majority of Seasons Hospice patients are cared for in their home, whether that is a private residence, nursing home, assisted living facility or other residential facility. All hospice services are available to our home hospice patients within a 30 mile radius of Rochester, encompassing seven counties.

Where are hospice services offered?
Hospice services are offered wherever the patient calls home.  This can be in the patient’s private home, nursing home, assisted living facility, adult foster care, group home, Seasons Hospice House or wherever else they may reside.

How is hospice care paid for?
Services may be paid for by Medicare, Medical Assistance, private insurance or private pay. 

Who can make a referral?
Referrals can be made by physicians or other health care professionals, family members, friends, clergy or the person seeking care.  To make a referral please call Seasons Hospice at 507-285-1930.



Seasons Hospice House

What is the background of the Hospice House?
Seasons Hospice opened its residential hospice facility in 1997 with the distinction of being the first independent, Medicare/Medicaid certified facility in the state of Minnesota.  Originally built in 1952 as a home for the family of Dr. F. Edmund Donoghue, a Mayo Clinic physician and his wife Winifred, Seasons Hospice House is situated on 13 beautifully wooded acres in rural Rochester, Minnesota. Tour Our Facility

Each of the eight private patient rooms has a lovely view of the expansive green lawn surrounded by pine, oak and maple trees.  A large deck overlooking rose and flower gardens is available for patients and families to spend precious time together surrounded by nature’s beauty. 

The common areas of the house are large and open and have amenities like a fireplace and large deck.  A family room and kitchenette are available for visiting families and patients.  Volunteers play an integral role in the care and maintenance of the house and grounds, completing the circle of partnership with the community in bringing hospice care to those in need. 

What services are available to patients residing at the Hospice House?
Residential hospice services include a private room, handicapped accessible bath facilities, 24 hour nursing care, meals and snacks as desired, medication administration, assistance with personal cares, nurse call system, telephone service, cable television and wireless internet access.  Hospice House patients also have access to services provided by Seasons Hospice social workers, music and massage therapists, our chaplain and volunteers who provide hospitality, cooking, and patient visits.  The Seasons Hospice medical director regularly visits patients at the house.

How is care at the House paid for?
Payment for residential hospice care varies for each person.  Some private insurance companies provide for all or part of the care.  The Seasons Hospice staff can help patients and family determine payment specifics as they relate to their particular situation.  For patients that lack a payer source, there is a Patient Care Grant and Veterans Care Grant that may be available to offset expenses based on pre-determined criteria.

What services are available for managing grief?
We specialize in providing comprehensive loss, grief, and transition services.  The staff of licensed, master-prepared counselors and specially trained volunteers provides educational and supportive services and programs for the bereaved, those who care for the bereaved, and the community at large.
